- Once you have an appointment scheduled, please click on the FORMS link and download the forms. Please have these all filled out and submit them at least 12 hours prior to your appointment. If you cannot fill them out ahead of time, please fill them out at least 30 minutes prior to your appointment.
- Call to confirm your appointment the day before. If you need to cancel for any reason, we appreciate as much notice as possible (at least 24 hours).
- Your appointment will consist of meeting with your provider (1 hour).
- Be prepared to make a payment upon check-in. We understand that dealing with insurance can be confusing and frustrating, so we will attempt to contact your insurance to verify your coverage prior to your appointment, and obtain an estimate of the portion you’ll pay out-of-pocket.
