At Live Well Psychiatry, we are in-network with all major Idaho healthcare insurance plans, as well as several out of state plans. We also have deeply discounted rates for patients who would like to pay without using insurance (self-pay).

Which Insurances Are We In-Network With?

*Our office does NOT accept Medicare or Medicaid

When you hear that a healthcare provider “accepts” a specific insurance, it usually means that they are an in-network provider with that insurance. When a healthcare provider is in-network with an insurance carrier, it means that the provider has a contractual agreement with the insurance company to provide medical services at pre-negotiated rates.

Visiting an out-of-network provider, on the other hand, may result in higher costs, as the insurance company might cover a smaller portion of the bill or none at all. Always check your insurance company's provider directory or member portal to confirm if a provider is in network before scheduling an appointment.

Every insurance provider has several different plans. We recommend reaching out to your insurance to verify that our providers are covered by your specific plan. Live Well Psychiatry is in-network with the following insurances:

  • Aetna-PPO, Trinity  

  • Allegiance Benefit (Not if it’s Woodgrain SAHS) 

  • Allied Benefits – Only if Aetna is on the card 

  • Blue Cross-Connected Care 

  • Blue Cross Federal - has to see SPH or Steve for eval  

  • Blue Shield - Commercial (UUG- Univ of Utah - will show at top of card)  

  • Boon-Chapman Administrators  

  • ComPsych - uses BC ID# HWD  *Employer is: Hewlett Packard – can only see Dr. Hoopes and Joana PA-C

  • Employee Benefit Management Services (EBMS)

  • First Choice Health Network 

  • Health Management(HMA)

    Moda – Will have ‘Select’ on the card (any others we are out-of-network)

* If you have Medicare, and you still want to be seen at our office, you will be required to sign an Opt Out form, provided by the office and become a self-pay patient.

Which Insurances Are We Out-of-Network With?

While we are not in-network with these insurance companies, we may still be able to bill them. However, we are unable to be aware of what they will or will not pay for. If we are not in-network with your insurance, please reach out to them to find out if they will cover any of your appointments with us. Many of them we will not be able to bill because they will not accept claims from us. They may accept claims from you though. You will need to ask them to verify.

How Do I Find Out If Live Well Psychiatry is In-Network With My Insurance?

Unfortunately, we are unable to provide contact information for all the different health insurance companies. If you are looking for information on your insurance, please reach out to them.

You can call the phone number on your insurance card to verify provider coverage. While speaking with your insurance customer service, using the following terminology may help save time:

  • Does my plan cover “out-patient mental health” services?

  • Are my out-patient mental health services through you or another company? ex: Hewlett Packard employees usually have Blue Cross insurance but their mental health goes through ComPsych

  • Is Live Well Psychiatry an in-network provider with my plan?

    • Some insurance companies will need to verify each provider. Our providers are: Scott Hoopes MD, Joana (Gladys) Gonzales PA-C, Crystal Odegard PA-C, Jennifer Hua PA-C, Kristin Hawkins PA-C, Steve Barnum FNP-C.

  • What will be my co-pay or co-insurance with Live Well Psychiatry?
