Our secure Patient Portal is the most efficient way to communicate directly with your healthcare providers, schedule appointments, make payments, and request prescription refills. Through the portal, you can:

  • Send non-urgent messages to your providers.

  • Complete your appointment rating scales.

  • Request prescription refills as well as see which pharmacy your prescription was sent to.

  • Access your medical records.

  • Review test results.

  • Make a payment as well as see your payment history.

  • Schedule an appointment with your provider.

  • Log in with your credentials. If you're a new user, please contact our office to set up your account.

How to Access on Your Smart Phone:

There is not a separate app for our patient portal that you can add to your smart phone. However, you can add a shortcut to your home screen. You can google how to do that for your specific phone. Here are some examples of how that shortcut link looks on different phones:

Best for: Refilling prescriptions and scheduling appointments for current patients.

For quick and convenient service, current patients can use text messaging to:

  • Request prescription refills.

  • Schedule or reschedule appointments.

How to Use:

  • Please include your full name and date of birth for verification purposes.

  • Please also let us know what you are texting about, e.g. “schedule next appointment,” or “question on last bill,” or “refill Zoloft,” etc.

  • Pharmacy Transfer: If requesting a pharmacy transfer, please leave the patient’s name, date of birth, which medication(s) you want transferred, and the full address of the pharmacy that you are transferring to.
